Advanced Nurse Practitioners

ELMS has a key role in the Pennine Lancashire health economy, supporting primary care and deflecting patients away from secondary care.   As a Community Benefits Society, ELMS primary focus is on patient care in a primary care setting, but works in close partnership with providers across the healthcare system.

We are looking for experienced ANP’s for evening and weekend cover to improve access for patients to the Out of Hours services and facilitate a clear patient pathway through primary, community, secondary and tertiary care where required. Shifts available are home visits, face-to-face surgery shifts, or telephone triage, evenings and overnights.

This role will appeal to ANP’s who would want the opportunity to develop and improve access to and provision of services. Ideally, candidates should have extensive experience working in Primary Care and a passion to keep care focused on the individual.

Successful applicants will provide clinical care to patients as they present via the out of hour’s service working with the OOH team to delivery high calibre patient care.

You will undertake assessment of presenting complaints and offer advice, health education, treatment and referral, as appropriate, including the prescription of medicines where clinically required working independently, managing autonomous caseloads. The post holder will demonstrate critical thinking in the clinical decision-making process.

In order to work at this level, the NMC requirements for advanced practice must be met.
