The ELMS Navigation Hub is a service that provides a direct interface with 111 supporting at risk patients, GPs & other health and social care professionals. It is staffed by Nurse Advisers and deals with cases that are more complex.
The team have access to a comprehensive directory of services for Pennine Lancashire and can make direct transfers or broker referrals to facilitate personal care and medicine arrangements for those patients at risk of hospital admittance.
The Nav Hub works with Integrated Neighbourhood and Locality Teams and with the Intermediate
Care Allocation Team (ICAT), the Intensive Home Support Service (IHSS), Reablement, Crisis Teams
and the Voluntary Sector
